Driver Report of Accident

Description:This is a WI state accident report to complete if you were involved in an accident that resulted in $1,000 or more of damage to property, OR anyone was injured, AND a law enforcement officer did not complete an accident report.

Retail Security Guidebook

Description:This Word document is a book developed by the Sturgeon Bay Police to help retail stores understand internal theft, external theft, check fraud, and countermeasures to these business-damaging problems.
Suggested Use:This guidebook should be used by reatil business owners to better understand theft and fraud, and ways to prevent these crimes. It can also be used as part of an employee training program to prevent shoplifting, or to help train loss prevention officers.

Description:This Word document is a report developed by the Sturgeon Bay Police to help retail stores record facts related to retail theft apprehensions.
Suggested Use:This report shoould be used to document facts every time a store apprehends a shoplifter. It should also be kept on file to use for testimony in court.

Vacation Watch Request Form

Description:This form can be used to request a vacation watch from the Sturgeon Bay Police. If you are going away on vacation, the Sturgeon Bay Police will provide checks on your home or property free of charge, and will contact you if there are any problems with your home or property.
